Okay, so I've got it back to the stage where if I start playback from MyMovies with some titles (for example The Meg), playback doesn't start, and if I go to MPC-BE and look at recent file, the file that was attempted to play is there, and if I play it it plays fine.

Some title won't play (The Meg), others will play fine (Lucy). All UHD Bluray folders. Very consistent. The same titles that don't play do play with MPC-HC (even from MyMovies) or jRiver.

I looked at the event log and here is what I get when MPC-BE fails to launch:

I've deleted all the LAV versions except the one use in the jRiver plugins folder. I've re-registered both the 32bits and 64bits .ax in these folders, to make sure these were the one used. MPC-BE is set to use external filters, and points at LAV video/audio/splitter/splitter source, same as MPC-HC. Both use madVR, but I get the same issue if I set the renderer to EVR-CP, so not a madVR issue.

The exception code is always the same, but the fault offset and Faulting process ID change every time.
