After your rebuild completed did you take a look at the new, rebuilt files before you burned them? I always do that now, so that I can spot this type of problem before I spend a blank BD-R on them.
So far I've only had the one bad burn out of 21 and I am looking at the log files now. Maybe I had a flaky blank BD?
