well, if sup is not supported by CCExtracton, then go with srt (as you have done yet).
You have said that after the error msg, SubtitlesCreator loads the srt file. Is it displayed correctly? Can you see all the subs?

what is the Display Aspect Ratio (DAR) of your avs file? Remember that AviSynth script does not have any DAR flags, so DAR is equal to clpi.Width/clip.Height.
So far only one audio track is supported. Multi audio tracks feat is in the ToDo list.
